Obtain an Operator's (Bartender's) License

To obtain an Operator's License (commonly called a "bartender's license"), complete the form in its entirety and have your signature notarized.  You must also include proof that you have completed a responsible beverage server training course or a copy of a current Wisconsin Operator's License.

Submit the completed form and proof of training, with a $30.00 application fee to the Village Hall.
