To obtain a license, please complete the application form by downloading the form PDF and returning it to the Village office with payment.  Please note, a copy of your dog's or cat's rabies certificate is required as well.

Register in person at the Village office or mail completed applications to:

If mailed, a tag will be mailed to you within five to seven business days.
